Insurance

For Provider Information:

Billing:
In-Network: Claims filed electronically.
Out-of-Network: Superbill receipts provided for filing with policyholder’s claims.
Patient Financial Responsibility: Guarantor remains financially responsible for payment of all services provided, including all non-covered charges under the patient’s health plan, and/or any out-of-pocket fees.
Note: Guarantors may elect to sign a credit card authorization form to be kept on file that would authorize collection of payment for these charges.
